Registration - 2008

 

LATE REGISTRATION begins June 1st and ends August 15th

 

How To Register - There are 2 Methods (Registration events have concluded)

1.  Obtain a registration form (click here to download form)  Or pick up a form at the True-Value Hardware store on Missouri Flat Road.

 

2.  When filling out the registration form:

Press hard for the copies. Or if you downloaded the form, be sure to provide the original plus an additional copy.

Make certain that all the information you supply is correct.

Provide the doctor's contact information.  (IMPORTANT!)

Please provide at least 2 phone numbers and an e-mail address.

Including an e-mail address is highly recommended, so that you can receive soccer club news in a timely manner.

Sign the form.

3. Items to mail: Please read these instructions carefully, as any deviation will delay the processing of your child's application.

Photo copy of player's birth certificate (ONE req'd for recreation play, and TWO required for select play).  Must be issued from the county of birth (not from the hospital of birth).

 

NEW FOR 2008:  IF the player was registered with Sierra Gold Soccer as a U8 or older player during the 2007 season, you do NOT have to resubmit a Birth Certificate!

 

1.5" x 1.5" headshot of player being registered (no sunglasses or hats please).  Important: the head/face portion of this photo should occupy at least 1"x1" of the photo space.  Please do not deviate from these specifications or we will be calling you for a new picture, delaying your application.

 

Registration cost per player until (postmarked by) May 31st is $85.  After that, a $25 Late Fee will apply, increasing cost to $110 per player.  A personal check, cashier's check, or money order for $110 made out to:

Age Groups
Please refer to the Age-Group Matrix to determine which Age-Group your child will play in.

 

Mixed Gender Teams
In accordance with CYSA guidelines as well as board member review and subsequent approval, the U6 age group is the only place where mixed gender teams will exist.  That means that from U8 and up, all teams will be formed as either all boys or all girls.

 

Late Registration

Late registration begins on June 1st and closes on August 15th by US mail.  If possible, you want to register as soon as possible, prior to June 1st to avoid Late Fees and to allow your player to be placed on a team within your geographic region.  Late registration is a time consuming process, and can result in complications if the correct age-group team already has a full roster.  In additions, any registration received after August 1st is at risk of the player missing some games because they may not receive their player pass in time, which is required for game play.

 

Late Fees
Beginning June 1st, registration applications will require an additional $25 late fee + $85 registration fee = $110 total.  We do accept late registrations, but please understand that late applications require additional processing by our hard working volunteers.  This is due to deadlines handed down by our parent district EDYSL club registrar.  Late fees generally do not apply to U6 players or to families that recently moved into the area.

 

Bounced Check Policy
A certain number of Registration checks that the club receives will be returned for Insufficient Funds.  In these cases, we cannot accept another personal check.  Two options are available: 1) cash or 2) Money Order.  In either case, you need to submit the original amount plus $6 to cover the bank fees that the club is charged.

 

Multi-child discounts
NEW FOR 2008, multi-child discounts have been suspended, due to budgetary constraints.

 

COUPON BOOKS benefit the club's Field Fund
NEW FOR 2008, each registered player will receive ONE coupon books (instead of two), which may be sold to the general public for $10 (you keep the money).  100% of Coupon Book sales goes towards our Field Fund our field maintenance efforts.  Coupon books will be distributed by the team's Coach at the first game of the season.

 

Registration Refunds-Recreation Players
Prior to August 1st, refunds can be processed for a full refund.  After August 1st but prior to September 1st, refunds will be minus $35.00 League Fees that we submit to begin team formation.  After September 1st, refunds will be minus $35.00 League Fees and minus $10 Field Fund (for the Coupon Book).  After the first game of the season (September 6th) refunds are not available. NOTE:  All refunds will be processed at the first board meeting after the first game of the season is played.
